Our REACH program brings a healthcare provider to your home to deliver the same level of care you would receive in the office. They work closely with your primary care physician and other members of your care team to meet all of your healthcare needs. They can even become your primary care provider if you don’t already have one.

REACH provides house call services to patients 50 and older who are unable to leave their home without assistance or require a movement device (such as a wheelchair or walker).

REACH can prevent unnecessary visits to the emergency room, decrease the length of stay at short-term rehabilitation facilities, and reduce the risk of hospital admission.

REACH integrates resources from across Hartford HealthCare's entire network of care, including the Center for Healthy Aging, Hartford HealthCare at Home, Behavioral Health Network, and more.
