Medicare Annual Wellness Visit

If you have traditional Medicare or a Medicare Advantage plan, you can get one Annual Wellness Visit (AWV) every 12 months — at no cost to you.

What is an Annual Wellness Visit?

An Annual Wellness Visit is different from a physical exam. Unlike a physical, an AWV does not include a hands-on exam or treatment for existing medical problems. The focus is prevention and your future health.

  • A physical exam is hands-on, includes a full body check, and is usually done by your primary care provider.
  • An Annual Wellness Visit (AWV) is a one-on-one conversation with a healthcare provider to review your overall health, lifestyle and safety. Together, you’ll create a personalized plan to help you stay healthy in the long run.

Who Provides the Visit?

Most annual wellness visits are completed by a nurse practitioner or physician assistant from your care team. They are specially trained for these visits and work closely with your primary care provider, sharing information to keep your care coordinated.

What’s Included?

During your AWV, your provider will:

  • Screen for depression, anxiety, memory issues and safety risks
  • Review your medical, surgical and family history
  • Go over all your all medications and supplements
  • Discuss your diet, exercise and lifestyle habits
  • Talk about advance care planning (like a living will or healthcare representative)
  • Create a schedule for preventive screenings and vaccines

Do I Need to Come in Person?

Most visits are offered virtually — by video or phone — so you can join from the comfort of home. Adult family members are welcome to participate, too.

Who is Eligible?

You qualify if you:

  • Have traditional Medicare or Medicare Advantage.
  • Have not had an annual wellness visit in the past 12 months.

Note: Medicare Advantage patients may be eligible for both a physical exam and an AWV each year.
