How to Request Your Records
You can request your records in one of two ways:
- Through MyChart: The fastest way to request or send your records is through your MyChartPlus account
- By Mail: Download and complete the appropriate release of information form, then mail it to the Hartford HealthCare Medical Group office where you received care.
Please allow up to 30 days for processing your request.
