Resource for Elderly and Chronically Ill Homebound

Our dedicated team of nurse practitioners and physician assistants are available to provide the highest level of quality care in the comfort of your own home.

What is REACH?

Our REACH program brings a nurse practitioner or physician assistant to your home to provide the same level of care you would receive in the office. They work closely with your primary care physician and other members of your care team to meet all of your healthcare needs. They can even become your primary care provider, if you don’t already have one.

What can REACH do for you?

• Prevent unnecessary visits to the emergency room
• Decrease the length of stay at a short-term rehabilitation facility
• Reduce the risk of hospital admission

Who is REACH for?

REACH provides house call services to patients 50 and older who:

• are unable to leave their home without assistance, OR
• require a movement device (such as a wheelchair or walker)

REACH services include:

• Physicals & other examinations
• Diagnosis & treatment of illness
• Medication prescription & management
• Memory assessments
• Home safety evaluations
• Education and support
• Transition of care
• Urgent care within 1–3 days
• Virtual care visits available

• Home Care such as Home PT/OT
• Behavioral Health
• Community Network
• Integrated Care Partners

Should you require a stay in a hospital, skilled nursing facility or short-term rehabilitation center, our entire system of medical experts in all areas of health and wellness are available to you as you transition seamlessly from one area of care to another.
